We offer a variety of special health services and exams to folks at community pricing appropriate for the occasional a la carte style of standalone care.

VISITS ARE BY APPOINTMENT

Our Services are by Self-Pay Only.

We are out-of-network from insurance plans and so we do not directly participate with, nor bill insurance. We are an independent direct primary care private practice (DPC), which follows a self-pay membership model. You can use insurance for what is covered for external services but not for care directly received from the clinic.

Any service delivered directly by our practice cannot be billed nor submitted for Medicaid, Medicare, TRICARE, or other similar government-sponsored health coverage program for reimbursement.

Rescheduling and Cancellation Policy:

We do understand that plans can change. Should you need to reschedule, please let us know ASAP, preferably at least 24 hours in advance. If you cancel after submitting your pre-visit documentation or if we must cancel due to lack of response, or a no show occurs, 50% of your service fee will be refunded. This helps cover the time and care our team dedicates to preparing for your visit.

Occupational & Other Health Services

Audiogram

Community Pricing: $135 Members: $0 This is for pure tone audiometry with threshold determination. ANSI certified equipment used. Estimated visit time: 45 min

Basic Occupational/Employment Physical, Return to Work Physical

Community Pricing: $175 Members: 1 included/yr Please contact us to check for availability for your particular exam immediately after booking your appointment. - Includes urinalysis and point-of-care blood sugar testing if needed - if a Chest X-ray is needed, a referral order will be placed for outside imaging for discounted pricing. Community Pricing: - Additional $50 fee if advanced in-house testing such as Spirometry (PFT), standard resting ECG, Color Vision screening, or Threshold audiogram is indicated for clearance. Discount-package for 2 or more of these tests is available at $100. Members: - 1 basic occupational/employment/return-to-work physical included per year, $85 per exam after this Estimated visit time: 45 - 60 min

DOT/CDL Medical Certificate (inquire about us coming to you!)

Community Pricing: $150 Members: 1 included/yr - additional $10 for audiogram if indicated for clearance based on examination findings and requirements. We are by appointment only, or you can message our number about potential availability off the booking calendar. We don't pile up a waiting room this way and we require drivers to complete the DOT history portion ahead of time so as to streamline the visit and catch any potential snags that can be addressed ahead of time. Members: - 1 DOT/CDL certificate exam included per year, $85 per exam after this. Estimated visit time: 45 - 60 min

US Coast Guard Merchant Mariner Physical Exam for Mariner Medical Certificate (CG-719K)

Community Pricing: $180 Members: 1 included/yr Members: - 1 USCG certificate exam included per year, $85 per exam after this. Estimated visit time: 45 - 60 min

At-home Sleep Study

Community Pricing: $425 Members: please reach out to discuss options This pricing includes the initial visit, the study, and a follow up visit. The initial visit includes the screening and home study kit, as well as execution of the study. The follow up visit after the study includes counseling, the study report, and study interpretation. If indicated by your study results, a prescription for a CPAP machine and/or sleep medicine specialist referral will be included with our service; you may choose to arrange obtain CPAP equipment by self-pay or potentially through insurance coverage. A sleep specialist review of your study to support possible insurance coverage for a CPAP machine will also be provided at no additional cost if needed. Estimated visit time: two 45 min visits

Fitness to Dive Physicals / Medical Clearance

Dr. Marshall is a Diving Medical Examiner (DMAC Level I) and is able to determine fitness to dive clearance for commercial, sport, scientific, other related public service, and recreational divers. Please contact us to confirm availability for your particular exam immediately after booking your appointment. - A Diver may be referred when medically appropriate for additional testing and evaluation such as Bone and Joint X-ray survey, Cardiac Stress Testing, ENT specialist evaluation, or additional special labs or testing when indicated on initial clearance or by age group, and is responsible for associated additional costs and appointment arrangements. - Referral orders will be placed for completion of any needed outside imaging and labs, which are available at discounted rates Commercial and Public Service Diver: A Flat fee covers as indicated: spirometry, standard resting ECG, standard Labs package, color vision screening, Audiometry, Chest X-ray, Physical exam - Community Pricing: $875 - DPC Members: $425 Scientific & Research Diver: A Flat fee covers: spirometry, standard resting ECG, standard Labs package, Audiometry, and Physical exam. Additional fee if Color Vision screening or CXR are needed. - Community Pricing: $650 - DPC Members: $325 Recreational Sport, Scuba, or Free Diver: - Community Pricing: $250 + if medically indicated: outside labs or imaging at discounted pricing, additional in-house advanced testing if indicated (such as Spirometry (PFT), Color Vision screening, or Audiometry) at the standard cost as listed on the website. Standard resting ECG included in pricing if indicated. - DPC Members: $100 + if medically indicated: outside labs or imaging at discounted pricing, additional in-house advanced testing if indicated (such as Spirometry (PFT), standard resting ECG, Color Vision screening, or Audiometry) at the standard member cost as listed on the website. Estimated visit time: varies based on needs, please plan for about 3 hrs for advanced dive exams.

Sports Physicals

(Other than for Youth Sports)

Special Vision Testing

(Ishihara, HRR)

Respirator Clearance Questionnaire only

Community Pricing: $85 Members: $0 Clearance decisions are followed according to OSHA Guidelines. Additional cost will apply if a physical exam is required for clearance based upon review of the completed respirator questionnaire. Estimated visit time: 30 min

Respirator Clearance Physical

Community Pricing: $175 Members: please see service description This includes a Respirator Clearance Questionnaire and will be prorated into the pricing for this respirator physical if the clearance questionnaire was completed with us and directly resulted in the requirement of this physical exam. - if imaging such as Chest X-ray is needed, a referral order will be placed for outside imaging for discounted pricing. Community Pricing: - Additional $50 fee if advanced in-house testing such as Spirometry (PFT) or standard resting ECG is indicated for clearance. Discount-package for 2 or more of these tests is available at $100. Members: - 1 respirator clearance physical included per year, $85 per exam after this Estimated visit time: 60 min

Additional Special Services

Acute / Urgent Care Visits

Community Pricing: $300 Members: please see service description - Acute care visits are by appointment only, even if same-day. Please contact us regarding your medical concern and for scheduling. Messaging our phone number works best. Medical Emergencies: We are not an emergency department. If you feel that you are experiencing a medical emergency please immediately call 911. - Additional costs for some take-home supplies such as DME (crutches and splints) may apply.

Youth Sports Physical

Community Pricing: $125 Members: please see service description - Includes urinalysis and point-of-care blood sugar testing if needed. - if imaging such as Chest X-ray or other outside workup is needed, a referral order will be placed. Community Pricing: - Additional $25 fee if advanced in-house testing such as Spirometry (PFT), standard resting ECG, Color Vision screening, or Audiometry is indicated for clearance. Discount-package for 2 or more of these tests is available at $50. Members: - In-house services included with membership. Estimated visit time: 45 min

Osteopathic Manipulative Treatment

OMT treatments are conducted as medically indicated for diagnosed somatic dysfunctions. OMT is available for L&I-covered work injuries by Dr. Marshall. .

Pessary Services

The Pessary Boutique is available for pessary fitting and physician-guided maintenance care for symptomatic pelvic organ prolapse. .
