Your membership covers your in-person and virtual visits, without added copays:

Comprehensive wellness and preventive care
Management of chronic conditions
Hormone replacement therapy as appropriate
Acute care
Typical in-clinic procedures (such as laceration repairs, joint injections, nerve blocks with great discounts on medication)
Travel, recreation and wilderness adventure medical planning
Access to our pediatric emergency physician to triage and address acute concerns
Access to significantly discounted or fully covered pricing for special services.

Discounted Partner Services:

Benefit from great self-pay DPC pricing for many partnered services when compared to insurance-based and cash-pay rates, including but not limited to:

Controlled Medication Prescriptions

For patient health safety and as general Practice Policy, Mountains Meet Medical does not prescribe opioids nor benzodiazepines for routine long-term/chronic therapy.

Our Services are by Self-Pay Only.

We are out-of-network from insurance plans and so we do not directly participate with, nor bill insurance. We are an independent direct primary care private practice (DPC), which follows a self-pay membership model. You can use insurance for what is covered for external services but not for care directly received from the clinic.

Any service delivered directly by our practice by law cannot be billed nor submitted for Medicaid, Medicare, TRICARE, or other similar government-sponsored health coverage program for reimbursement.

Membership Plan Selections

Pearl Membership

$185/month

Comprehensive primary care.
Specialized medical services & exams included, or at significantly reduced pricing.
3 OMT sessions annually, discounted at $150 per additional session thereafter.

Sapphire Membership

$250/month

Comprehensive primary care.
Specialized medical services & exams included, or at significantly reduced pricing.
6 OMT sessions annually, discounted at $100 per additional session thereafter.
Includes routine annual and monitoring labs!*

*Annual and monitoring labs are ordered as clinically appropriate for ongoing preventive and chronic disease management.

*Examples of routine labs included for monitoring:

Diabetes
Cholesterol
Thyroid
Chronic kidney disease
Vitamin deficiencies
Cervical cancer screening
Routine medication monitoring
And more

*Examples of labs not included under membership:

Specialty testing
Genetic testing
Hormonal labs (initial workup)
In-depth diagnostic workups
Acute and emergency diagnostic workups
Outside specialist-requested labs

Family Membership Discounts

After the 1st (primary) member, each additional family member receives an incremental $25 off monthly pricing:

To a minimum of $85 per member as part of the Pearl plan, or

To a minimum of $150 per member as part of Sapphire plan.

Family members must reside together.
Youth under 18 years old require enrollment under a family membership, with an adult primary member.

Intensive Support Membership

For patients whose care benefits from extended physician involvement

— in clinic and beyond.

All Ages (per person): $550/month

This membership option significantly expands capacity for patients whose care involves distant home-visits, substantial coordination, time-intensity, or ongoing heavy administrative health documentation. It is designed to include the support of:

Comprehensive primary care.
Specialized medical services & exams included, or at significantly reduced pricing.
3 OMT sessions annually, discounted at $100 per additional session thereafter.
Includes routine annual and monitoring labs as described under the Sapphire membership.
Up to 3 scheduled medically appropriate in-home visits annually, within 100 miles of the clinic location.*

*Extended Distance Travel for home visits (more than 100 miles distance from the clinic location) and supplemental in-home visits beyond the 3 annually included incur additional hourly travel time fees.

*In-home visits must be chaperoned.
