Appointment Booking Confirmation:
Please note that submission of all intake components at least 24 hours prior to your appointment date is required to finalize your appointment booking (associated intake forms, payment). If left incomplete, we may need to reschedule or cancel your appointment.
Rescheduling and Cancellation Policy:
We do understand that plans can change. Should you need to reschedule, please let us know as soon as possible. If you cancel less than 24 hours prior to your confirmed appointment or we must cancel due to lack of response/no show/further requested forms are still left uncompleted, only 50% of your service fee will be refunded. This helps cover the time and care our team dedicates to preparing for your visit.

Audiogram
Community Pricing: $135 Members: $0 This is for pure tone audiometry with threshold determination. ANSI certified equipment used. Estimated visit time: 45 min

Basic Occupational/Employment Physical, Return to Work Physical
Community Pricing: $175 Members: 1 included/yr - Includes urinalysis and point-of-care blood sugar testing if needed - if a Chest X-ray is needed, a referral order will be placed for outside imaging for discounted pricing. Community Pricing: - Additional $50 fee if advanced in-house testing such as Spirometry (PFT), standard resting ECG, Color Vision screening, or Threshold audiogram is indicated for clearance. Discount-package for 2 or more of these tests is available at $100. Further Policies: If your exam requires specific forms by your employer, please plan to attach the forms to the intake questionnaire that you will access as a part of the intake forms. You may be notified of additional reports, letters, or documentation needed for submission prior to your visit day based on review of your medical history. If this is requested, please ensure that we receive them well in advance of your appointment day so that there is time to review them and avoid rescheduling or cancellation to maintain the quality of care you deserve. US Coast Guard Merchant Mariner Physical Exam for Mariner Medical Certificate (CG-719K)
Community Pricing: $180 Members: 1 included/yr Members: - 1 USCG certificate exam included per year, $85 per exam after this. Please come prepared to your physical exam with your unexpired government-issued photo identification and any usual required documentation for your certification. If you wear contacts for vision correction, please bring a case with contact solution, as you will need to remove your contacts to test uncorrected vision as part of the vision exam. Estimated visit time: 45 - 60 min

DOT/CDL Medical Certificate
Community Pricing: $150 Employer-sponsored booking: scroll down to find the "Employer-Sponsored DOT Booking" button. Members: 1 included/yr - additional $10 for audiogram if indicated for clearance based on examination findings and requirements. We are by appointment only, or you can message our number about potential availability off the booking calendar. We don't pile up a waiting room this way and we require drivers to complete the DOT history portion ahead of time so as to streamline the visit and catch any potential snags that can be addressed ahead of time. At-home Sleep Study
Community Pricing: $425 Members: please reach out to discuss options This pricing includes the initial visit, the study, and a follow up visit. The initial visit includes the screening and home study kit, as well as execution of the study. The follow up visit after the study includes counseling, the study report, and study interpretation. Plan to schedule your follow up visit at least two days after you plan to perform your sleep study to allow for time to analyze your study. If indicated by your study results, a prescription for a CPAP machine and/or sleep medicine specialist referral will be included with our service; you may choose to arrange obtain CPAP equipment by self-pay or potentially through insurance coverage. A sleep specialist review of your study to support possible insurance coverage for a CPAP machine will also be provided at no additional cost if needed. Preparation for your evaluation: See the Step by Step Guide for how the study is completed so that you may look it over ahead of time and identify any issues you foresee arising that may prevent your completion of the exam successfully. For example, you will need internet access and a smartphone or device such as a tablet that can download and use the WatchPAT app from the App Store or Google Play Store; you can test this ahead of time now. You will need power to keep your device plugged in the entire time while you are undergoing the study. Fitness to Dive Physicals / Medical Clearance
Dr. Marshall is a Diving Medical Examiner (DMAC Level I) and is able to determine fitness to dive clearance for commercial, sport, scientific, other related public service, and recreational divers. As noted above: we do NOT provide formal drug screening as part of these services. - A Diver may be referred when medically appropriate for additional testing and evaluation such as Bone and Joint X-ray survey, Cardiac Stress Testing, ENT specialist evaluation, or additional special labs or testing when indicated on initial clearance or by age group, and is responsible for associated additional costs and appointment arrangements. - Referral orders will be placed after the exam for any needed outside imaging and labs (including chest x-ray), which may be available at discounted or self-pay rates if not already covered by your flat fee pricing. Commercial and Public Service Diver: A Flat fee covers as indicated: spirometry, standard resting ECG, standard Labs package, color vision screening, Audiometry, Chest X-ray, Physical exam - Community Pricing: $875 - DPC Members: $425 Scientific & Research Diver: A Flat fee covers: spirometry, standard resting ECG, standard Labs package, Audiometry, and Physical exam. Please contact us to check for availability for your particular Sports physical exam before booking your visit. - Includes urinalysis and point-of-care blood sugar testing if needed - if imaging such as Chest X-ray or other outside evaluation is needed to determine clearance, a referral order will be placed. The sports participant is responsible for associated additional costs and appointment arrangements. Such referral orders will be placed after the exam for any needed outside imaging and labs (including chest x-ray), which may be available at discounted or self-pay rates if not already covered by the pricing described on this listing. Respirator Clearance Questionnaire only
Community Pricing: $100 Members: $0 Clearance decisions are followed according to OSHA Guidelines. If a physical exam is required for clearance based upon review of the completed respirator questionnaire, your payment will be applied toward the additional cost of the clearance physical, as listed on the website. Respirator Clearance Physical
Community Pricing: $175 Members: $0 This includes a Respirator Clearance Questionnaire and will be prorated into the pricing for this respirator physical if the clearance questionnaire was completed with us and directly resulted in the requirement of this physical exam. Pulmonary Function Testing (Spirometry)
Community Pricing: $145 Members: $0 Please come to your evaluation wearing loose, unrestricted clothing to minimize barriers to taking deep breaths. Ensure that you remove gum and/or loose dentures from your mouth. Piercings in the mouth do not generally need to be removed. **If you are feeling sick on the days prior to and the date of this evaluation we advise that reschedule for another date when you are feeling well. Please contact us right away about this. This is for the reason that feeling unwell will likely interfere with your testing results, mis-representing your usual pulmonary function. If you are pregnant in the last trimester, we will also postpone testing. If your blood pressure is significantly out of control, or you are not medically stable in any way, we will need to postpone this testing. Estimated visit time: 45 min

Acute / Urgent Care Visits
Community Pricing: $350 Members: please see service description - Acute care visits are by appointment only, even if same-day. Please contact us regarding your medical concern and for scheduling. Youth Sports Physical
Community Pricing: $125 Members: Included with membership. Please see service description & contact the clinic for scheduling. - Includes urinalysis and point-of-care blood sugar testing if needed. - if imaging such as Chest X-ray or other outside workup is needed, a referral order will be placed. Community Pricing: - Additional $15 fee per any advanced in-house testing service needed for clearance based upon history and/or physical, such as Spirometry (PFT), standard resting electrocardiogram (ECG), Color Vision screening, or Audiometry. Youth Sports Physical Form: Please be prepared to attach a photo or pdf copy of the completed youth sports physical form required by the school. This is likely the official Washington Interscholastic Activities Association (WIAA) Form, as found at https://www.wiaa.com/forms/ for the WIAA Medical History Form - go to the Health & Wellness Forms section. Please download the Medical History Form from the WIAA website and read through the questions carefully while completing the form.
